As the 2009 Fall Sports seasons quickly approaches, the time to make preparations is upon us.  In order for students to be ready for practice on the first day, several administrative tasks must be attended to.  Listed below are the important matters should be on every athletes list of things to do.

FHSAA participation documentation is required for every student athlete.  There are two forms necessary.  A Preparticipation Physical Evaluation form (EL2) must be filled out and signed by a physician or chiropractor before the first day of practice in addition to a Consent and Release from Liability Certificate (EL3).  Be advised that no other physical form filled out by a doctor may be accepted and a new physical is required for each year of athletic participation.  If you have already received a physical for the upcoming year, you should bring this form into the doctor and ask him to sign off on it.
Student-Athletes should be ready and equiped for the first day of practice.  The dates are as follows:

Varsity Soccer – August 12 – 3:00 PM – All athletes should have shin guards with appropriate NOCSAE safety labels.  Cleats as well as athletic shorts and shirt are also required.
Varsity & JV Volleyball – August 12 – 3:00 PM – All athletes should have knee pads and appropriate athletic attire.
Volleyball Grades 5-8 – August 17 – 3:00 PM – Again, all athletes should have knee pads and appropriate athletic attire
Athletic Fees will be as follows.  Students participating on Varsity, Junior Varsity, and middle school teams – $100.  Elementary Teams – $50. These fees will be billed to your account on the 15th of September.  A student is considered to have participated in a sport when they have dressed for the first competition.
